Old question but I'll answer anyway. No, at least in stores. They tend to come, Omron, etc with a standard arm cuff only or standard plus x-large & only provide the opportunity to purchase the smaller 6-8" size for $19.99 (or as low as $13-$14 on Amazon). Some brands offer smaller cuffs for as little as $7-8. I tried NET searches for small arm, thin arm, 6-8" arm BP monitors & always landed on smaller cuffs sold w/o the monitor. One might email the various manufacturers & ask if they will sell direct to you a monitor /w 6-8" cuff instead of making you pay extra when you don't need the reg size and/or if they actually sell such a monitor but I didn't find it.
