It is a cutout in the hard center, it is a nub in the soft outer case that pushes against the buttons. When aligned correct, it should be easy to use

Please try again later.I have used Otterbox covers on 46 phones (personal, family and business phones) over the past six years, It appeares the Otterbox design anticipates using the button covers intact. Cutting them out would require accessing the buttons through a thick case and would seem inconvenient. if your case is out of warranty, you might try the modification.
