No, the phone screen isn't covered by clear plastic with this case. However, the case does protect the screen if the phone is dropped on a side edge or corner. You could add gorilla glass to your phone if worried about scratches.

No it does not. However the cover on the phone is pretty tough, so I do not think you need to waste your money on a screen protector that doesn't fit well and pops off.
(I learned that one the hard way)

The new defender does not come with the plastic shield, do to the curved glass on the phone. The shield has to be purchased separate and it is very delicate.
