I had this issue once. I cleared all my paired devices,rebooted my phone, and started over. That worked for me.

Tap the voice button and then say "pair" or press and hold the call button until you hear "pairing." Activate Bluetooth on your phone and set it to search for new devices. Select "PLT_Legend." If your phone asks for a passcode, enter four zeros (0000); otherwise, accept the connection. NOTE: Not all phones prompt for a passcode. Once successfully paired, you will hear "pairing successful."
