A:AnswerSorry. I don't know about ability of PNY Nvidia Graphics Card GT710 to fit into HP slimline. It's small and compact indeed but I put this card in a Dell Desktop PC CPU box that is rather large w/ plenty of room. The best thing about bestbuy; however, is their return policy. If it doesn't fit return it directly to a nearby Best Buy. You can't beat the price of this video card and it did the job I needed it to do when I replaced my existing and older Graphics Card in my Dell Precision 390. Good luck!
A:AnswerYes. The GT710 should work. I looked up the Lenovo m91 and it should have the following slots:
Two full-height PCI
One full-height PCI Express x1
One full-height PCI Express x16
The Nvidia Quadro NVS 285 will be removed and replaced with the GT710.
A:AnswerYES - I use it with three different monitors - if your monitors only have DVI and VGA input ports, you'll need to get an HDMI to DVI adapter (HDMI Female on one end, DVI male on the other) because the third port on the card is hdmi. Those adapters are about $7 on amazon.
A:AnswerThanks Don,
I checked the hp website and it looks Like the 6204y supports PCI Express x16 graphics cards. The Asus motherboard should support pci express. Here is what I saw:
Also supports PCI Express x16 graphics cards*
If you check your computer and open it up there should be a free slot for pci express.
Below is the website for your computer specs.

Hope this helps.
