“I am fairly OK with installs if it is not too complicated. How hard is the install? Also do we need to do some leveling with spirit levels on the ground? Should we buy a mat to put under the treadmill to give extra support on carpet? thanks”

It's not complicated. It just takes two people and some time. The unit has feet on it to adjust for leveling. A mat is always recommended to protect the carpet. The belt wears down eventually as you walk on it and it helps to keep that from getting into your carpet.
