“I have almost 39" between the left wall and the counter top but in all the photos I see that the fridge is sticking out the full depth of the doors or about 4.5". Is that necessary so it doesn't hit the wall or counter?”

I have the exact same clearances. The door swings and counter interference was the first things the installers checked once it was in place.
