I bet you are using ARC or e-ARC
This seems to be a "bug" or "feature"
When changing audio to TV speakers,
the remote volume works perfectly.
It isn't the remote, the button, or the user.

And again, seems to be only when using eARC

Hi KWilson, Many of the buttons on your remote have multiple functions. For example, the Volume button is moved UP/DOWN to adjust volume, and to MUTE the sound, you need to PRESS it in. If that same button is pressed for > than 1 second, the Accessibility Shortcuts appears. ^Val

You have to push the volume bar up or down from the top or bottom of the bar, and Press down on it to mute or unmute.
