“Do you have to take the four screws out of the back off the tv first to mount it or does it just use the spacers and the screw? What’s keeping the screw intact”

Yes, you have to take the screws out to mount and the type of mount you use will determine whether it needs spacers or not. Everyone I've mounted needed spacers though.
