• can't speak to this phone in general, but there is zero reason it wouldn't, and every smartphone i have ever purchased from att (iphone 4/5/5s/6+/ and Note 4) have all had sims already in the box.

• Yes it does. It has a smaller sim card that the micro sim of previous generations.

• At Best Buy and most other vendors, a SIM card will be included at no charge because you can't activate the phone without it. If someone tries to charge extra for a new SIM card, I recommend choosing a different vendor.

• Yes. My phone came with an AT&T Sim card. You also get instructions from best buy on how to activate it from home if you want.
