No, the main character is not trans, but there are connections to him, and something is going on in the town. This is a remake of the original Silent Hill 2 on PS2 and Xbox in 2001 and features plenty of shockful, scary moments that are NSFW. Please note that while SH2 doesn't feature a trans, MC Japan has always been accepting of transgenders, and games have featured trans characters going back to the 80s.
