Nothing if you had electricity to power in all speaker locations! Personally for rear speakers we didn’t have ceiling rear electricity so we utilized a Sonos amp and passive speaker which was expensive but less than getting electricity to these locations.

In order to set up a great sounding Sonos 5.1 surround sound system with ceiling mounted speakers you would need a Playbar, Playbase, or Beam for your front 3 channels (Left, Right, and Center), an Amp to power the in ceiling speakers or two ceiling mounted Sonos Ones for your rear channels (rear left and rear right), and a Sonos Sub for your bass (the .1 in 5.1).
