• No, but you have to have one of the newer controllers with the 3.5mm headphone jack port.

• If you have the controller with the 3.5mm jack, than no. 3.5mm is the normal size jack that most headphones fit in (like apple earbuds).

• NO, i only like using the adapter when i want little more volume, if you have one already, you should try it and see if you can hear a difference.
