Internet access is only required to set up the console, play games online, and use online services. Otherwise, it may be used offline without an internet connection.

Only if you wish to engage in PVP games with others online. You can play campaigns and split screen. I would personally recommend internet though, as the gaming experience is something you don't want to miss.

No, but some games require you to have Xbox live which requires internet. So it depends on what games you get.
