Not necessarily, if you already have Netflix, Hulu, etc. account you can log into your existing accounts through the tv and no Roku account is necessary, but you could set one up if you wanted it all billed out of the same place. It's totally up to you.

Yes. You have to create a Roku account (which is free) in order to use Roku products. You only need one account for all of your devices, like TVs or streaming sticks. You will need a credit/debit card for apps or content purchase, but you will not be charged anything unless you buy. You can also set a password to prevent children buying unauthorized content. Once you have set up the Roku account just sign in to Netflix, Amazon or any others (assuming you have a membership with them).
