Yes. 3 outlets with back up and surge. 3-4 more with just surge.

Yes, the product has a 3 prong plug. one hot, one return, and the ground. Standard 120 AC voltage configuration.

The one I purchased does have a 3 prong plug. I would suggest having your power receptacle ground verified if your concern is about grounding. If the receptacle ground isn't correctly configured or missing, then I doubt having a 3 prong ground chord from the UPS wouldn't be of any value or protection.

yes I would check teh wiring in your house to make sure the ground and polarity are correct.
