“Does this watch have a call 911 feature to use if you fall?”

You would have to use the emergency feature on the watch

Yes it does but it’s not 100% reliable.

It has a feature that you turn on or off. When you fall, it pops up and asks if you are ok and if you say no or don’t respond it will call your ICE contacts and then 911
