Yes it does come with sim card its already inserted, yes its one time payment $29.99 to purchase the phone, but you must choose a plan in order to have minutes on the phone in order to talk or text. I personally suggest $35 a month plan, which I use. You can activate it through that phone or online the instructions are all on the box and inside the box.
