It is based on you body frame. For a Side Sleeper: If you wear a medium shirt, then a 2.0. If you wear a Large or XL shirt, then a 3.0 For a Back Sleeper with a back problem, it may depend on the back problem, BUT if you like a more elevated feeling to your neck and shoulders, then a 2.0. If you like to sink in a little more, but still have good support, then a 1.0
