You get a cable card from your cable company. There is a slot it slides into. 
Comcast charges me 7.00 a month for the cable card in my area.

So with tivo you rent a cable card from your cable provider it is usually cheap.. Now if you are replacing one box a single tivo bolt is great but if you want to replace multiple boxes you need to purchase an extender known as the tivo mini. (Regular mini or the vox mini). Example i baught a bolt vox and 2 minis for 2 extra tv's. I saved $80/month. There are many different tivo models to choose from mine for example can be used with cable or an antenna.
