If you are paired to the headset and listening to music, you can switch the audio output in the lower right on the iPhone. A:AnswerI also use the large and at times I would like it to fit a bit tighter but I have never had it fall out in over two years of use. It is so comfortable that I have to check to see if I remembered to remove it before I step into the shower. I would welcome an XL size myself.
