Ron, I think I understand your question. You would connect all of the necessary components (face cam, HMDI cables, stream deck, etc.). Open the stream software of your choice, (I use OBS SE.LIVE). Once open, add the Elgato into a scene as both a video and audio source. You would then add the face cam, microphone, etc., into the scene. Once you have done this you should be able to see and hear the gameplay on the second monitor. All you need to do then is click start streaming! I have attached a photograph to help you understand.

Sorry, there was a problem. Please try again later.If I'm reading this correctly yes, you can you just have to connect the capture card to the monitor that's playing the game and connect the USB to your laptop or pc.
Sorry, there was a problem. Please try again later.Yes. Just make sure you have OBS or StreamLabs running on the PC that is capturing. Do all of this setup before adding the PS5 and the HD60X as a source. Also be sure to connect your microphone to that same pc. Audio capture of your voice won’t come through the PS5. Good luck!
