Yes it is online play only. If you are on ps4 or xbox you will need either ps+ or xbox live to play. Although once in the game you can play solo, or join up in teams with people you meet.

Yes. And they do server maintenance periodically. When this happens, they give you a 30 minute countdown timer in the upper right of the screen to give you time to finish up a quest or whatever you're doing before it shuts down. Then all you do is log back in and it'll switch you to a different server so you can continue your game.

yes and no ... as the cover states, you must have an internet and playstation plus/essential subscription, but you dont necessarily have to play with a group or with another player. You can explore the game solo and complete quests/missions.
