Do you have to remove phone from case to charge? null

I have heard that some you do. Mine has a Speck Candyshell Grip case and I have no problems.

No it didn't work with case on. I grounded a little at a time the male connector on radio. now phone charges perfectly.

Probably not. I have a slim-profile case, and mine fit right on without even having to remove the rubber pad (which is designed to be removed for thicker cases). I would imagine most cases would then fit if you do remove the pad.
