It feels very flat, I sometimes use a small pillow. If you use a pillow though the massagers can't get to your neck.

You can use the head cushion to reduce the intensity of kneading on your neck
and shoulders. (We recommend using the head cushion.)
The backrest and back cushion are connected by a zipper.
