Probably not if the pills are small. The blade sits up from the bottom of the grinder cup, so if they aren't big enough to touch the blade they can't get ground up.

Once it got here, I took one look at the raised blade and thought "Oh No," but to my surprise, it ground two teaspoons of flaxseed into a coarse meal. I think the friction makes the small items float upwards. I'm not sure how it did it, but it worked just fine and flaxseeds are itsy bitsy.
