“Every time when powering on receiver and cable box volume comes in and out, video is also affected. The only fix is to switch output from cable/sat to dvd or any other output and wait a few seconds and then go back to cable/ sat.”

Make sure you have a fiber optic cable from tv to the receiver along with your HDMI cable. This fixed that problem for me...
