• Mine didn't come with an adapter. It's purely a USB connected headset. No 3.5mm at all.

• The headset uses a USB connection (not a USB jack), so you'll only need an adapter if your port isn't a USB.

• Not sure how to answer that. What do you mean by adapter? It comes with the USB cord, which helps if you want the surround sound and higher fidelity processing. Otherwise, you can alternatively use the 1/8" standard earphone cord to use it as standard earphones. I found that the extra processing was unnecessary and not that great, so I hope that helps.
