“This is a 4 Input to 1 output switcher correct? The picture shows one Input port on the front... odd there is not a pic of the backplane....... Also, can this output 3440 X 1440 at 60Hz?”

It has 1 Input on the front , 3 input on the back, and 1 output on the back.
