This would depend on the level of humidity in your home and the temperature you have your fridge and freezer set at. Freezer temp should be between 0 to 5 degrees F & fridge should be about 40 degrees F. House humidity should be approx. 45%.
This could also be caused by moist items being put in the fridge.

If all of these settings are ok I would suggest having someone look at it to make sure water is not be introduced into the fridge from the icemaker or some other source.

Depends on humidity and how often/how long the refrigerator doors are open.
