Some themes may be dark, e.g. death and the coming end of the world. Middle school age would be more than fine; I have encountered numerous players in this age group.

Unfortunately there all the other options were even worse - had to buy FIFA separately to have something nice to play. Would never have gotten the PS4 if FIFA were still available for Wii. I did read up on the uncharted 4 after hearing that this game was more puzzle oriented than violent. Reviews at a catholic website (USCCB) indicated excessive violence with some redeeming insights on family relationships and value of telling the truth. You can find better material on the PS4 if you look for it (stick to sports) otherwise I strongly recommend moving to a more family friendly platform.
