• Yes it is compatible with the PlayStation slim I do want you to know it will take up one of your USB ports you'll plug your HDMI chord from your PlayStation 4 to the unit and then from the unit you'll plug another HDMI chord into your TV and yes it does include a second HDMI chord

• Yes it works with all models. You will have a hdmi going from your ps4 to the pass through / transfer box and then and hdmi cable going from the pass through box to the tv. It comes with one cable and you should already have a cable that came with your system so you will be good to go
