The main pivot bolt seems to have some tolerance, allowing the mounted TV to tilt downward. The washers don't have enough friction to hold the TV's weight from tilting. Get a 14mm or 9/16 socket, and a 6mm (I think) allen key. Use them to tighten that pivot bolt and nut. Then gently un-tilt the TV to see if it doesn't droop back down. This was just my problem with a Samsung QN90B 65" which is approximately 55lbs. If your TV is tilting up or down on the pivot bolt, this solution should work either way.
