• I have used it on an IPhone 5 with a Lifeproof case, but you have to press much harder on the screen.

• Yes I found that it is best to pull the clear plastic off of the defender case and then put the glass film over the phone. This product has saved my phone countless times

• Don't have an otterbox but I am sure it does. It leaves enough edge so any case should fit fine
