A: As long as it's a standard 3.5 mm jack for the PS4 headphones, then yes it will work with the Xbox one adapter. If it is a wireless headset it will not.
A: This controller will accept any headset with a 3.5mm jack. If by, "doesn't work good" means that the audio is low and doesn't get very loud then this will help with that. It has a built in amp powered by 2 AA batteries. Headsets can get much louder with this attached to the controller.

A: To my knowledge, there is no Discord app on the PS4 so you'd have to use the remote play app on your home PC to play the PS4 version of the game and use Discord at the same time. If you're just wanting to use the PS4 in-game voice chat feature then you'll still need a microphone as this is just an audio amplifier. I use this to power a Sennheiser gaming headset that already has a microphone.
