• No, it's an optional purchase. I do recommend it since it is such a high purchase though.

• No you don’t HAVE to but it wouldn’t be a bad idea. I’m not a huge fan of protection plans for phones because I’ve paid for them and never used them but with this device it’s cheaper to buy AppleCare upfront or with monthly payments than it is to pay to repair the screen out of pocket. Damage your phone and it needs to be repaired? It will cost you over $500 according to Apple website. You have 14 days after purchase to decide if you want it if you want to do monthly payments. 60 days if you want to pay for it in full.
