No monthly fee unless you contact your wireless carrier and activate a cellular plan for your watch. If your watch is unlocked, you do not need a carrier to sync your phone. If you have an iPhone, it should be compatible with an Apple Watch. Just make sure that you have an iPhone that is compatible with your Apple watch. If you decide to activate the cellular feature later, contact your preferred carrier and ask if they have cellular service for an Apple Watch. I hope this helps.
