Does the door/exterior get hot too? null

Yes, the exterior of the Breville® BOV450 Smart Oven® Mini, as well as the door, can get very warm while in operation.

The oven does get hot but we have ours under a tall upper cabinet with 7.5 inches clearance to bottom of cabinet with no issues.

Yes the heat from the interior does transfer to the exterior, you can use the top to keep other dishes warm, but I wouldn’t permanently store anything on the top.
