It is recommended you have your TV or monitor connected directly to the video cards ports and not the integrated graphics ports. Also, make sure you have only one cable connected to each TV or monitor. Connecting more than one cable from your computer to one TV or monitor will cause windows to think you have more than one TV or monitor connected.
