Every time I have had this issue it's always been caused by a pair of shorts with a drawstring. The drawstring gets caught on the lip of the lint trap and spins around and around twisting the whole time. Eventually it twists everything in the dryer into one large rope as you described. It only happens occasionally. I have no solution for this issue other than making sure the lint trap is securely seated in place.
