“Why does my LG Tone Ultra SE keep telling me it is out of range when it is right next to my phone? This only happens when it is turned on, but not in use. It does not disconnect when in use.”

More than likely it's a defective device. I've owned mine for almost 2 years and have yet to have a problem with them.
