This is an Ethernet port to plug into a non-Wifi enabled device - an (older) printer perhaps. You can also use this extender as a Wireless Access Point. Search Google for the user manual

Theoretically there is a loss of performance when connecting to something via wireless. So when you have a device that connects Wirelessly to your primary WiFi and you connect wirelessly to that device you have the potential to lose 2Xs the performance speed. The cord also allows you to use the extender on a device that might not have wireless capabilities.

The ethernet plug on the side is if you want to hard wire something like a computer, switch, gaming console, or media streaming device. It's a nice feature since you will always achieve the fastest speeds by using a wired connection.
