No it’s just comes with the case and belt clip. But you can purchase one separately.

It does not come with a screen protector, probably due to the curved screen on the S9. You will need to buy that separately. Shop around since they can be pricey. I recommend the SpectreShield screen cover.

No... You must buy a separate screen protector. I think that Otterbox makes one, but I took advantage of Best Buy's discount by buying three accessories at the same time. I went with the Zagg protector which is acceptable, but I don't love it.

No it does not. I have not put one on mine yet as some do not fit with the otter box and Samsung does not have one at the present time. I am just going to purchase a film to protect it from scratching it. I do like the otter box.
