If it's in your ear, powered on & synced up to your phone, you still need to accept the incoming call to get things started - unless you set it up to auto-answer, assuming the same conditions are true. The auto-answer setting can lead to missing calls, especially during times when the ear piece may not be in your ear; but it's still synced up to the phone. Even though this may happen, the volume on the ear piece is quite capable, so unless you're in the car with the music turned all the way up; you should be able to pick up on the sound of your caller's voice.

The headset will always alert you of an incoming call when you have it on your ear, on most cases the phone will also ring unless it is in silent or vibrate mode. If the headset is not in your ear it will not alert you of an incoming call.
