It means the remote has a mic in it. You can change services with it by saying Hulu, HBO, etc. after pressing the microphone button, but that's about as useful as it gets. It could theoretically work with apps as well, but it they would have to be programmed to use it. I also have a couple of Ultras. No difference in remote functionality, except the Ultra has a headphone jack on the remote.
