A:AnswerIt appears to be by device for the promo. So two devices, two promo rewards. You go through the Shop Samsung app on the phone itself unless you ordered more than 5 devices. Samsung locks you out from computer registration if just a single device. Just make sure you have a picture or scan of the receipt to upload with your claim. When I got mine we bought two at the same time, one receipt. So far, both have been accepted for the promo.

A:AnswerNo it does not. You have to choose either the 360 cam or the 128GB card with the wireless charger. This can be done via the shop samsung app. Refer the website for further details
A:AnswerThe main issue you will have with an unlocked device is the SIM card, going from an old flip phone you are almost assured to have to get a new one. The card I was using with my Galaxy Note 3 was too old, form factor wouldn't fit (SIM cards are like SD now. They have a standard, mini, and micro size) so I had to get a new SIM card and activate that. Otherwise, shouldn't be any activation fees as the device is ready to use out of the box. I was without a SIM in my device for the first couple days while getting it set up, only had a prompt at the notification bar saying "no SIM card." Carrier won't be as helpful with device centric issues because it isn't "their phone" yet the freedoms of unlocked is completely worth it.
