• I have a Samsung Note 3 & paired it with that so i'm sure you can pair it with the Note 4, as long as it's a Samsung phone.

• It is compatible with the S5 so I see no reason that it wouldn't be compatible with the Note 4. There is a free Samsung App that you download to your phone then connect it by Bluetooth.

• yes its compatible with any samsung device that has the samsung s-health app on. It connects via bluetooth.
