You would definitely be required to reinstall Windows. New hard drives are always blank, no data, no OS.

Some PCs have multiple hard drives and the OS may be on one with data on another. If you don't fall into that camp then yes... OS does not ship with the hard drive.

If you're planning on adding a new hard drive, and want to have some good performance, I'd suggest purchasing this hard drive for storage, and the PNY 240GB SSD for just the OS. If you're not sure on how to set up partitions on your computer for multiple drives, then I'd suggest asking the Geek Squad to help you get set up. IMO, using the SSD for the OS, and a hard drive for storage is a huge improvement over just one drive for the OS and storage. I can boot up to Win 7 in under 15 seconds.
