• It's the same pen. It is the tablet size that is different. Medium active area is 8.5 in x 5.3 in. Small active area is 6.0 in x 3.7 in.

• The names are a bit misleading. The "Small" and "Medium" refer to the size of the tablet's work surface, not the pen itself. The Small is 12.6"x8.2" (6.2"x3.9" active area), and the Medium is 15"x9.9" (8.8"x5.5" active area). Hope this helps, and happy creating!
