Q: Will this fit a Cat 5 Connector?
A: No Cat 5 connection, only electrical outlets. Some surge protectors have that capability and some also have phone line/modem line protector functions, but not this one.
A: Yes, it has a fail safe.
Q: what is the UL 1449 Voltage Protection Rating (VPR)?
A: I could only find a suppressed voltage rating which was 400v (L-N, N-G, L-G) Hope this helps.

Q: If one (or more) outlet(s) were to fail, what indications indicate that?
A: Honestly, The best way for me to see if it is a wall outlet that has failed or if it is the unit outlet. is to plug it into a known functioning outlet. So if everything in the power strip fails, take a lamp for example and take it to an outlet in your house that you know works. If you plug in the strip and the lamp works, then just one by one go down the plug till the lamp doesn't light up. If the lamp lights up on every strip outlet, then it is most likely the wall outlet, if it fails on on of the strip outlets when testing you know it is the strip.
